Enhancing Integrated Care helps primary and community care delivery organizations strengthen integrated team-based care models, including virtual care, making access easier and easing pressure on emergency departments.

Healthcare Excellence Canada launched the Strengthening Primary Care in Northern, Rural and Remote Communities program (Strengthening Primary Care) to strengthen access to team-based primary care in northern, rural and remote communities.

Reimagining LTC supported long-term care homes to work together to build better care with and for people living and working in long-term care in Canada. The program helped participants address gaps in the safety and quality of care in long-term care, by providing more support to healthcare workers and building capacity to make care more person-centred.

The Cultural Safety Design Collaborative supported non-Indigenous organizations to address systemic racism experienced by First Nations, Inuit and Métis and improve cultural safety in the healthcare system.

Virtual care is growing rapidly across the country as more primary care providers use technology to deliver healthcare. That’s why Healthcare Excellence Canada has launched an initiative to help care providers and patients work together to ensure virtual care is provided in an appropriate, safe and equitable way.

EXTRA™ is Healthcare Excellence Canada’s team-based, bilingual, action-oriented leadership development program that supports ongoing system transformation. It focuses on building the capabilities of leaders to improve quality and safety in healthcare.

An awards-based quality improvement program supporting the appropriate use of antipsychotics through person-centred care in long-term care homes.

The Promoting Life Together Collaborative modelled a co-designed learning program, bringing together multi-disciplinary teams from across northern, rural and remote parts of Canada using a shared learning approach. The name of the collaborative was re-framed to focus on ‘life promotion’, recognizing the need to take a broader and more encompassing approach to the issue of suicide, to consider all aspects of one’s life and community wellness.

Healthcare Excellence Canada partnered with Canada Health Infoway, the College of Family Physicians of Canada and the Royal College of Physicians and Surgeons of Canada to spread the adoption of two proven innovations that had substantially increased access to primary care specialty services.
