Enhancing Integrated Care helps primary and community care delivery organizations strengthen integrated team-based care models, including virtual care, making access easier and easing pressure on emergency departments.

Healthcare Excellence Canada launched the Strengthening Primary Care in Northern, Rural and Remote Communities program (Strengthening Primary Care) to strengthen access to team-based primary care in northern, rural and remote communities.

The Canadian Northern and Remote Health Network (CNRHN) brings together senior decision-makers, leaders, policymakers and practitioners, to develop effective and sustainable solutions to improve healthcare and the health status of people living in northern and remote areas of Canada. Members enhance their leadership and improvement skills by sharing learning, innovative ideas, tools and resources.

The Nursing Home Without Walls program helps nursing and LTC homes support older adults to age in place with access to essential services and support.

This collaborative aims to improve access to equity-oriented palliative care with and for people experiencing homelessness or vulnerable housing.

Through the Enabling Aging in Place collaborative, 26 health and social service organizations worked alongside older adults, care partners and community partners to strengthen locally grounded, person-centred approaches shaped by lived experience and real-world conditions. Together, they advanced practical ways to bring care closer to home and support people to live safely and well in their communities.

Reimagining LTC supported long-term care homes to work together to build better care with and for people living and working in long-term care in Canada. The program helped participants address gaps in the safety and quality of care in long-term care, by providing more support to healthcare workers and building capacity to make care more person-centred.

The Cultural Safety Design Collaborative supported non-Indigenous organizations to address systemic racism experienced by First Nations, Inuit and Métis and improve cultural safety in the healthcare system.

Virtual care is growing rapidly across the country as more primary care providers use technology to deliver healthcare. That’s why Healthcare Excellence Canada has launched an initiative to help care providers and patients work together to ensure virtual care is provided in an appropriate, safe and equitable way.
