Across Canada, teams are developing and adapting approaches that support older adults to age in the places they call home.

This collection brings together promising practices that help improve access to care, strengthen community connections and support safe transitions across care settings. These approaches come from a range of initiatives, including Healthcare Excellence Canada’s Enabling Aging in Place collaborative, as well as other leading work across the country.

Instead of reinventing the wheel, this Quebec team adapted a proven model to their local context. In communities where the program has been implemented, the team has had a nearly 25% reduction in 911 calls, helping to lower costs and ease the strain on emergency services.

ACE Units: Supporting Safer Transitions Home

Being admitted to the Newfoundland and Labrador Health Services' ACE Unit is a completely different experience for older adults - the entire person is cared for, not just the medical reason that brought them into the hospital. This work is based on a proven method seen at other hospitals across Canada, and has patients feeling more confident when they are discharged.

Dementia Friendly Life Enrichment

No matter how much information you have, nothing can prepare you for living with, or caring for someone who lives with dementia. In Saskatchewan, a community program is helping people living with dementia stay connected, supported and engaged in their communities, ultimately delaying their entry into long-term care.
